  • Overview

    According to the prescribed policies and procedures of the organization including all applicable federal, state and local regulations and under the general supervision of the Agency Therapy Manager, the purpose of this position is to supervise, provide and direct physical therapy rehabilitation services in compliance with company and agency policies and procedures. A review of performance will be conducted on a yearly basis.


    • The PT is responsible for the evaluation and treatment of clients who demonstrate musculoskeletal deficiencies,
      neurological conditions, balance, gait and transfer difficulty, and additional deficits and/or conditions resulting
      in a functional decline. Also may include safety assessment and environment modifications for patient
      surroundings and home environment. Evaluation and treatment will only be conducted in accordance with the
      rules of the Occupational Therapy, Physical Therapy, and Athletic Trainers Board of the State of Ohio and the
      Ohio Revised Code, Chapter 4755.
    • The PT will be responsible for the supervision of physical therapist assistants in accordance with the Conditions
      of Participation and with the rules of the Occupational Therapy, Physical Therapy, and Athletic Trainers Board of
      the State of Ohio and the Ohio Revised Code.
      The PT will be responsible for the instruction, evaluation, plan development, and supervision of the home
      health aide when appropriate per the Conditions of Participation.
    • The PT may assist with community education to increase awareness of agency services and therapy benefits
      with referrals as necessary and appropriate to assist individual’s ability to remain in community dwelling.
    • The PT is responsible for the timely and accurate completion of all necessary documentation, which includes
      but is not limited to the following items:
      a. Daily Time Log
      b. Initial Evaluation/Assessment to include plan of care for physician’s approval with rehabilitation
      c. Daily Visit Note with progress summaries as appropriate toward plan of care
      d. Recertification with intermediate summary for plan of care
      e. Discharge Assessment and Summary
      f. Start of Care, Resumption, Transfer, Recertification or Discharge OASIS when appropriate
      g. All other required forms as designated by the agency and agency therapy manager
    • The PT will maintain progressive knowledge of the physical therapy field, attending related in‐services and
      continuing education programs.
    • The PT is responsible for attending all required facility meetings and in‐services, including, but not limited to
      patient staffing and interdisciplinary team meetings for coordination of patient care and staff
    • The PT will assist the agency in meeting the Conditions of Participation and other standards necessary to obtain
      and/or maintain state, federal, and other appropriate certifications.
    • The PT will employ and follow all appropriate agency infection control policies and procedures including OSHA
    • With respect to client rights, ensures all care is provided with respect and dignity for clients, reports all
      complaints made by clients and/or families to the therapy manager or other appropriate manager, reports all
      allegations of abuse, misappropriation of funds/client property and/or any other corporate compliance items
    • The PT is responsible for the appropriate management of clients on caseload and meetings to meet agency
      productivity standard of at least 75%.
    • The PT is responsible for the scheduling and completing of in‐services for agency staff education.
    • The PT will provide in‐service education programs for agency personal when indicated.
    • The PT will present herself/himself in a professional manner at all times including conduct, communication, and
      appropriate dress when representing the agency.
    • Adheres to departmental and agency policies and procedures.
    • Maintains responsible work attendance, communicating requested days off with at least 30 days advance
    • Other duties as assigned by the agency therapy manager and agency.


    Education: Bachelor of Science Degree in Physical Therapy
    Experience: Minimum of two years clinical experience
    Mental: Must have good communication, comprehension, and interpersonal skills. Must have the
    ability to speak, read, write and understand English.
    Skills: Must have working knowledge of Windows, Microsoft Office Suite, and World Wide Web. Must have good written and verbal communications skills.
    Licensure: Current Ohio License
