    According to the prescribed policies and procedures of the organization including all applicable federal, state and
    local regulations and under the general supervision of the Agency Therapy Manager, the purpose of this position is
    to carry out treatment plans and other rehabilitative services in compliance with company and agency policies and
    procedures. Physical Therapist Assistants will be supervised by a Physical Therapist, performing duties outlined by
    State Licensure Board, and will have documentation reviewed and signed by the Physical Therapist. A review of
    performance will be conducted on a yearly basis.


    •The PTA is responsible for the treatment of clients who demonstrate musculoskeletal deficiencies, neurological
    conditions, balance transfer and gait difficulty, and additional deficits and/or conditions resulting in a functional decline. May also include recommendations to supervising PT regarding safety and environment modifications for patient surroundings and home environment. Treatment will only be conducted in accordance with the rules of the Occupational Therapy, Physical Therapy, and Athletic Trainers Board of the State of Ohio and the
    Ohio Revised Code, Chapter 4755.
    •The PTA will be responsible for communicating changes in patient progress or status to supervising physical therapist.
    • The PTA may assist with community education to increase awareness of agency services and therapy benefits with referrals as necessary and appropriate to assist individual’s ability to remain in community dwelling.
    •The PTA is responsible for the timely and accurate completion of all necessary documentation, which includes but is not limited to the following items:
    a. Daily Time Log
    b. Daily Visit Note including objective summaries of patient progress as appropriate toward plan of care
    c. All other required forms as designated by the agency and agency therapy manager
    • The PTA will maintain progressive knowledge of the physical therapy field, attending related in‐services and continuing education programs, needed to maintain state licensure.
    • The PTA is responsible for attending all required facility meetings and in‐services, including, but not limited to patient staffing and interdisciplinary team meetings as needed for coordination of patient care and staff support/education.
    •The PTA will assist the agency in meeting the Conditions of Participation and other standards necessary to obtain and/or maintain state, federal, and other appropriate certifications.
    •The PTA will employ and follow all appropriate agency infection control policies and procedures including OSHA Guidelines.
    •With respect to client rights, ensures all care is provided with respect and dignity for clients, reports all complaints made by clients and/or families to the therapy manager or other appropriate manager, reports all allegations of abuse, misappropriation of funds/client property and/or any other corporate compliance items immediately.
    •The PTA is responsible for the appropriate management of clients on caseload and meetings to meet agency productivity standard of at least 75%.
    •The PTA will provide in‐service education programs for agency personal when indicated.
    •The PTA will present herself/himself in a professional manner at all times including conduct, communication, and appropriate dress when representing the agency.
    •Adheres to departmental and agency policies and procedures.
    • Maintains responsible work attendance, communicating requested days off with at least 30 days advance notice.
    •Other duties as assigned by the agency therapy manager and agency.Manager, Regional Rehabilitation Manager, and facility.


    Education: Associate Degree or equivalent in Physical Therapy from an approved curriculum.
    Experience: Minimum of two years clinical experience
    Mental: Must have good communication, comprehension, and interpersonal skills. Must have the ability to speak, read, write and understand English.
    Skills: Must have working knowledge of Windows, Microsoft Office Suite, and World Wide Web.
    Must have good written and verbal communications skills.
    Licensure: Current Ohio License
