  • Overview




    According to prescribed policies and procedures including all applicable state, federal and accreditation regulations and under the supervision of the SiteManager/Nurse, provides personal care to meet the needs of the clients, supports other staff in providing activities in an environment that facilitates and encourages each participant to function as independently as possible both within and outside of the Center. 



    • Assists clients with daily direct care needs that includes but not limited to, toileting, bathing, feeding, nail care, hair care and mouth care and informs the nurse when personal hygiene supplies are needed.  
    • Is observant of the client’s behavior (i.e. increased confusion, decreased appetite, frequency of urination, color/odor of urine) and reports to nurse and/or social worker.
    • Helps clients complete exercise regimens as prescribed by the physician, physical therapist, occupational therapist, speech therapist and also assists with transfers.  Aids clients around the Center and to vehicles.
    • Encourages and stimulates the client to become involved in activities appropriate to his or her individual needs and level of functioning and participates along with clients when possible.
    • Responsible for preparation of snacks, serving and feeding according to the CACFP required training.  Also, assists life enrichment staff with inventory and ordering of kitchen, cleaning, snacks, food and paper supplies. 



    • In conjunction with the life enrichment staff, is responsible for general cleanliness and orderliness of the Center.  Ensures that light housekeeping is done, supervising clients in these tasks when appropriate.
    • Attends required in-services and mandatory meetings. Fulfills at least twelve (12) hours of in-service training each year.
    • Attends and participates in regularly scheduled interdisciplinary team meetings to, follow up on changes and other communication, problem-solve, receive staff support and in-service education.



    • Demonstrates and maintains a positive, professional manner, manages time efficiently and effectively, promotes a team approach, participates in appropriate problem solving methods.


    • With respect to Client Rights, ensures all care is provided with respect and dignity for clients, reports all complaints made by clients and/or families to the appropriate supervisors, reports all allegations of abuse, misappropriation of funds/client property and/or any other corporate compliance items immediately.


    • Must adhere to all policies, procedures, terms and conditions set forth in the National Church Residences’ Employee Information Guide (EIG) as well as any facility policies including but not limited to corporate compliance, drug free workplace, safe work practices, all federal, state, local regulations and laws.


    • Performs other duties as assigned


    Education:       High school diploma or successful completion of GED.   


    Experience:     Prefer two (2) years experience in providing personal care to an individual. 


    Mental:            Must have good communication, comprehension, and interpersonal skills. Must have the ability to speak, read, write and understand English.


    Skills:              Desirable to have working knowledge of Windows, Microsoft Office Suite, and World Wide Web.


    Licensure:        Desirable to have a valid driver’s license.
