    Responsibilities

    ESSENTIAL FUNCTIONS

    • Coordinates communication with physicians offices/other referral sources to obtain orders, signatures and paper work as requested by the clinical team.
    • Responsible for completing client visits in the field as census dictates or as DCM assigns and entering documentation into EMR accurately and timely per policy.
    • Tracks/obtains clinical items as listed above to ensure timely and appropriate initiation/completion of care.
    • Responsible for making calls and following up with physicians, physician extenders, referral sources and other providers as requested by the clinical team to further the patient plan of care.
    • Data entry of items into the medical record to facilitate timely delivery of care.
    • Assists with audits as instructed.
    • Provides back up for Intake Coordinator and Scheduling Staff as needed.
    • Completes all clinical duties as defined in the State Nurse Practice Act.

     

    NON-ESSENTIAL FUNCTIONS

    • Participates in in-service programs and teaches other nursing personnel.
    • Completes other administrative functions as assigned.

    • Demonstrates and maintains a positive, professional manner, manages time efficiently and effectively, provides leadership as a role model to promote a team approach, collaborates and participates in policy creation and appropriate problem solving methods.
    • Must adhere to all policies, procedures, terms and conditions set forth in the National Church Residences’ Employee Information Guide (EIG) as well as any facility handbook including but not limited to corporate compliance, drug free workplace, safe work practices, all federal, state, local regulations and laws.
    • With respect to Client Rights, ensures all care is provided with respect and dignity for clients, reports all complaints made by clients and/or families to the appropriate supervisors, reports all allegations of abuse, misappropriation of funds/client property and/or any other corporate compliance, drug free workplace, safe work practices, all federal, state, local regulations and laws.
    • Performs other duties as assigned. 

     

    Qualifications

    Education:  Degree in nursing field.

    Experience: Minimum of two years experience, preferably in a physician’s office or acute care setting providing administrative support and triage

    Travel:  Some travel. Must have a valid driver’s license, automobile insurance and qualified to driver under the organization’s motor vehicle check.

    Mental: Must have good verbal and written communication, comprehension, computer and interpersonal skills. Must have the ability to speak, read, write and understand English.

    Licensure: Must have a current nursing license in good standing.

    Vision: Normal: Consistent with standard workflow.
